Is Anxiety Medication A Controlled Substance

How Are Controlled Substances Different From Other Prescription Medications

Oklahoma’s New Electronic Prescribing Law Takes Aim at Controlled Substances

The Controlled Substances Act controls certain substances and places those previously regulated under existing federal law into one of five schedules. At a high level, the categorization is based on how potentially dangerous they are. More specifically, it takes into account the substances medical use, the potential for abuse, and dependence or safety liability. This categorization of drugs has been carefully considered and is based on research from many different sources to determine the drugs potential harmfulness. Laws have been put in place to protect people from the harm they can do to themselves or others.

Positive Effect On Brain Cell Connectivity

Dr. Gobbis study discovered that low doses of LSD promoted new dendritic spines in rodents which are nerve branches that transmit electrical signals between nerve cells such as those found in the brain. These nerve branches can become damaged due to chronic stress, and evidence showed that LSD repaired these structures in the mice.

Dr. Gobbi told Medical News Today, We were surprised about the neuroplastic effects of LSD, in particular the augmentation of spines in stressed mice, which indicates that LSD can facilitate new synapses.

Dr. Gobbis next step will evaluate the mechanisms of action of psychedelic substances psilocybin and ketamine. Future studies will consider ayahuasca and MDMA to treat addiction, depression, post-traumatic stress disorder, and anxiety.

MNT spoke with Eric Hollander, MD, a professor of Psychiatry and Behavioral Sciences at Albert Einstein College of Medicine and the Psychiatric Research Institute of Montefiore Einstein in New York City, New York.

Regarding study limitations, Dr. Hollander stated:

The study did not assess for the effects of LSD on depression, did not see any anti-anxiety effects in mice who were not stressed, did not study effects in females and did not study effects of single doses of LSD.

This study used repeated dosing rather than intermittent dosing, and showed the prevention of the development of anxiety in mice, rather than treating individuals who already had an anxiety disorder.

Selective Serotonin Reuptake Inhibitors And Selective Serotonin Norepinephrine Reuptake Inhibitors

Due to their positive benefit/risk balance, selective serotonin reuptake inhibitors and selective serotonin norepinephrine reuptake inhibitors . During the first 2 weeks, adverse effects may be stronger. Initial jitteriness or an increase in anxiety symptoms may occur, which may reduce the patients’ treatment compliance. Lowering the starting dose of the antidepressants may reduce these adverse effects. A review of studies in depressed patients suggested that SNRIs may be less well tolerated than the SSRIs. However, according to clinical experience, tolerability may differ among patients, and it is also possible that an individual patient may experience less adverse effects when switched from an SSRI to an SNRI.

Some SSRIs and SNRIs are inhibitors of cytochrome P450 enzymes and hence may interact with other psychopharmacological drugs and medications for medical illnesses. After stopping treatment with an SSRI, withdrawal reactions may occur. However, these are much less frequent and severe than the withdrawal reactions observed after terminating benzodiazepine treatment. These adverse reactions may be more frequent with paroxetine than with sertraline or fluoxetine.

Development And Maintenance Of Comorbid Anxiety And Substance Use Disorders

Research and clinical experience also reveals that when anxiety and substance use disorders co-occur, these disorders are functionally intertwined in both the development and maintenance of comorbidity. As detailed in the Figure, 3 primary pathways to comorbidity have been suggested from the culmination of studies to date.6 These pathways include: 1) a self-medication pathway wherein an anxiety disorder leads to a substance use disorder 2) a substance-induced anxiety disorder pathway and 3) a third variable pathway .

The Role Of Medication In Anxiety Treatment

When youre overwhelmed by heart-pounding panic, paralyzed by fear, or exhausted from yet another sleepless night spent worrying, youll do just about anything to get relief. And theres no question that when anxiety is disabling, medication may help. But are drugs always the best answer?

Many different types of medications are used in the treatment of anxiety disorders, including traditional anti-anxiety drugs such as benzodiazepines and newer options like SSRI antidepressants . These drugs can provide temporary relief, but they also come with side effects and safety concernssome significant.

They are also not a cure. In fact, there are many questions about their long-term effectiveness. According to the American Academy of Family Physicians, benzodiazepines lose their therapeutic anti-anxiety effect after 4 to 6 months of regular use. And a recent analysis reported in JAMA Psychiatry found that the effectiveness of SSRIs in treating anxiety has been overestimated, and in some cases is no better than placebo.

Whats more, it can be very difficult to get off anxiety medications without difficult withdrawals, including rebound anxiety that can be worse than your original problem.

The Controlled Substance Act Of 1970

For many decades, the United States has fought what is often termed a war on drugs. Recognizing the potential that certain medications have for abuse and dependence, Congress enacted the Controlled Substances Act as part of the Comprehensive Drug Abuse Prevention and Control Act of 1970. Over the years, the Act has had several revisions including:

  • The Psychotropic Substances Act of 1978
  • The Controlled Substances Penalties Amendments Act of 1984
  • The Chemical Diversion and Trafficking Act of 1988
  • The Domestic Chemical Diversion and Control Act of 1993
  • The Federal Analog Act
  • The Methamphetamine Precursor Control Act

The CSA mandates that manufacturers, distributors, pharmacies and healthcare providers diligently ensure the safe and efficient delivery of controlled substances identified within five schedules under the Act.

Treatment Of Gad In Older Patients

With the exception of GAD, anxiety disorders are less common in patients over 65 years of age. Therefore, only a few studies for the treatment of GAD have been performed with older patients. Controlled studies have shown the efficacy of duloxetine, venlafaxine, pregabalin, and quetiapine in patients over 65 years old. In the elderly, an increased sensitivity to drug side effects and interactions must be considered, including anticholinergic effects, risk of orthostatic hypotension and cardiovascular events, risk of falling, and paradoxical reactions to benzodiazepines.

In the elderly, effect sizes for CBT tend to be somewhat smaller than those found in mixed-age populations. A meta-analysis of studies with older adults with GAD showed superiority of CBT to waitlist or treatment-as-usual conditions but not to active controls .

MAOIs are a class of antidepressants, also called Monoamine oxidase inhibitors. These drugs are considered quite powerful antidepressants. The effects that quetiapine has on the body cause many to ask if the medication is an MAOI. When looking at a question about the Seroquel drug class, it is important to note that this medicine isnt classified as an MAOI. Similar to MAOIs, however, drinking on Quetiapine can lead to serious adverse effects.

The Difference Between Prescription Drugs And Controlled Substances

Not all prescription drugs are controlled substances. Antibiotics, for example, can only be obtained by prescription but are not regulated like Xanax is. Controlled substances are specific drugs that can lead to dependence and addiction.

In 1951, an amendment to the Federal Food, Drug, and Cosmetic Act was passed that defined which drugs required a doctors approval and which did not. The Controlled Substances Act of 1970 determined which substances came with criminal charges when used outside of prescription guidelines.

Special Benzodiazepine Risk Factors

Anyone who takes benzodiazepines can experience unpleasant or dangerous side effects. But certain individuals are at a higher risk:

People over 65. Older adults are more sensitive to the sedating effects of benzodiazepines. Even small doses can cause confusion, amnesia, loss of balance, and cognitive impairment that looks like dementia. Benzodiazepine use in the elderly is associated with an increased risk of falls, broken hips and legs, and car accidents. Long-term benzodiazepine use also increases the risk of Alzheimers disease and dementia.

People with a history of substance abuse. Because theyre physically addicting and on their own and dangerous when combined with alcohol and other drugs, anyone with a current or former substance abuse problem should use benzodiazepines only with extreme caution.

Pregnant and breastfeeding women. Benzodiazepine use during pregnancy can lead to dependence in the developing baby, with withdrawal following birth. Benzodiazepines are also excreted in breast milk. Therefore, pregnant women need to have a thorough discussion about the risks and benefits of these medications with their prescribing doctor. If medication is necessary, the goal is the smallest effective dose.
