How Pas Can Help Manage Patients With Mental Illness

Some of the most challenging patients for primary-care physicians are those with mental illness. Hereâs how PAs can help you care for them.

Some of the most challenging patients for primary-care physicians are those with mental illness. Mental health issues are complex, time consuming, and often difficult topics to broach.

I know personally the challenges physicians face in treating patients with mental illness. When I graduated from the PA program at Gannon University, I was hired to be a PA for psychiatric services in a small hospital, an outpatient clinic, and multiple drug and alcohol treatment centers. It was a big step for me as a new clinician.

Because of my solid, general medical education and training, and a supervising physician who trained me on the medical/psychopharmacology connection, I was quickly able to begin effectively caring for these complex patients. PAs work in collaboration with physicians. Although I worked autonomously most of the time, a psychiatrist was always available when I had questions.

PAs can also provide significant value to a general medical practice. PAs are educated and trained to conduct comprehensive medical and mental health assessments, through interviewing and physical examination techniques. Because of this training, they are often the ones to assess behavioral health concerns such as depression, anxiety, addiction, and other common mental health or psychiatric issues.

Treating Anxiety In The Primary Care Setting

Best Medicine To Treat Anxiety

Anxiety is an emotional and physical health problem marked by feelings of fear, unease, and worry. Sometimes it can be so severe that it make us feel physically unwell. The source of these feelings is not always known. It may involve a past trauma or injury, and sometimes it stems from a fear about something uncertain in the future. Treating anxiety in the primary care setting is perfectly doable, but not for patients looking for a quick fix.

A patient might say to me, I need something for my anxiety. They might have had prescriptions in the past for quick-acting medicines such as xanax, ativan, valium, or klonopin. Those are benzodiazepines, medicines that should only be prescribed with great caution and awareness of the risk for tolerance and dependence.

In some cases these medications are excellent options for treating anxiety but they have limitations. As a primary care nurse practitioner I want to move beyond treatments that seem quick-and-easy, but dont heal over time. The use of these quick-acting medications is an example of seeking an external locus of control over ones emotional state. While taking a pill might be quick and easy, making a lasting and effective change in the dynamic of anxiety takes time and patience.

When we develop the ability to recognize harmful self-talk, we can practice changing the way we speak to ourselves each of us can exert powerful restorative effects on our emotional and physical well-being.

Who Can Prescribe Antidepressants

Many people who face depression dont know what they can do to address their depressive symptoms. In fact, some people dont even know that treatments for their depressive feelings exist. When I experienced my first episode of deep depression, I was totally unaware of what could be done to help eliminate this unwelcome feeling. At the time, I was only 14 years old and had zero knowledge that antidepressants even existed.

The initial suggestion from my family was to go on a natural antidepressant called St. Johns Wort. Unfortunately we never even went to the store to buy the supplement. I had no idea that doctors prescribed pharmaceutical antidepressants to treat mental illnesses such as depression and anxiety. My parents ended up doing some internet-research and dragged me to the doctor for an antidepressant prescription.

Who Cannot Prescribe Antidepressant Medications

Licensed mental health professionals can help to provide alternate treatment for depression and anxiety, like talk therapy. However, many of these professionals cannot prescribe medication.

But in some cases, these professionals can make a recommendation to a physician or psychiatrist who can prescribe medication.

Collaborating With Other Health Care Providers

While FNPs are able to diagnose and treat non-complex mental health issues like anxiety or depression, they are not trained in psychotherapy. Collaboration is integral to the FNPs promotion of mental health whether that be referring someone dealing with anxiety to a clinical mental health counselor or connecting a patient with severe mental health issues to a psychiatrist. In order to promote mental health in their patients, FNPs develop robust networks of mental health care providers they can refer patients to, including psychologists, psychiatrists, psychiatric nurse practitioners, licensed clinical social workers and clinical mental health care providers.

Additionally, if a patient is being seen by a specialist for a physical condition, the FNP may find it beneficial to discuss the patient’s overall health both physical and mental with that provider to ensure the most comprehensive, holistic care possible. Or if a medication regimen that tends to work for an issue like depression does not seem to be working for a patient, an FNP may want to talk to another provider who can advise on another protocol to try.

Here are some of the medications you can expect your therapist, doctor or nurse practitioner to recommend:

  • Buspirone: Buspirone may be used to treat generalized anxiety disorder. After staring this medication, it usually takes a few weeks to start feeling better. It can be a better option for individuals with chronic anxiety, as it is less addicting than benzodiazepines.

Those with social anxiety disorders may take beta blockers to help control the anxiety stemming from being in a crowd.

What Does Prescriptive Authority Mean

The Nurse Practitioners prescriptive authority refers to the ability to prescribe medication. Nurse Practitioners can prescribe medication in all 50 states, although the independence with which they can prescribe drugs, medical services, or devices can vary depending on the state. The prescriptive authority for Nurse Practitioners is essential for healthcare practice and service. That prescriptive authority provides quality and cost-effective healthcare for patients who need fast, reliable, and accessible care in their communities.

The Nurse Practitioner prescriptive authority has been around since New Mexico first approved it in 2002. Soon after, Louisiana, Illinois, Iowa, and Idaho passed approvals. The American Association for Nurse Practitioners recommends that NPs not be restricted. Instead, they encourage NPs to prescribe medications, devices, medical equipment, supplies, and other healthcare services for patients. But, the AANP endorses oversight by the state board and the training, education, and certification credentials.

The Potential Of Post

Post-masters certificate programs teach nurses specialized knowledge in different health care areas. The programs provide nurse practitioners with excellent opportunities to increase their autonomy and expand into areas such as pediatrics, family health, womens health, and adult-gerontology.

Not all MSN programs lead to nurse practitioner certification. MSN-prepared nurses who arent nurse practitioners can complete a post-masters certificate to qualify to become nurse practitioners. Regis provides such a track for students earning a post-masters certificate, enabling those with MSN degrees to become nurse practitioners in as few as 20 months. MSN-prepared nurses who are currently nurse practitioners can pursue a post-masters certificate and expand into other areas.
