Resolving anxiety quickly should be the first goal of treatment in patients with anxiety and depression. Once this objective is accomplished, patients are much more likely to remain compliant with their antidepressant regimen and to continue their therapy for the full duration necessary to achieve complete remission of their depression.

One choice for pharmacotherapy in patients with comorbid anxiety and depression is the combination of a BZD or buspirone with an antidepressant. Alternatively, some patients may respond to antidepressant therapy alone, provided a drug that is effective in treating both disorders is used. Because the traditional anxiolytics will have little if any effect on depression, an adequate course of therapy with an antidepressant is imperative. On the other hand, in patients treated with an antidepressant plus an anxiolytic, the clinician may consider tapering the anxiolytic slowly, then discontinuing it when the symptoms of acute anxiety have resolved. Clinicians should choose a drug with a favourable tolerability profile and the least potential for drug interactions. Several antidepressants are potent inhibitors of cytochrome P450 enzymes.9 Combining more than one serotonergic drug, including multiple antidepressants, St John’s wort and some analgesics such as tramadol, can give rise to serotonin syndrome. A high index of suspicion is needed for patients who present with hypertension, hyperthermia, autonomic signs and hyperreflexia soon after starting, adding or increasing the dose of a serotonergic drug.10 Discontinuation syndrome is more common with some antidepressants such as venlafaxine and paroxetine.

Most patients with anxiety, and especially those with health concerns, for example in generalised anxiety disorder and panic disorder with or without agoraphobia, are highly sensitive to the physiological effects of medication. Adverse effects commonly seen when commencing antidepressants, such as nausea, headache and dizziness, may be misinterpreted as signs of serious physical illness or impending loss of mental control. Hence, the increased anxiety often observed when starting SSRIs may reflect a combination of normal physiological effects, heightened cognitive symptoms of anxiety as a result of fears about the seriousness or permanence of these adverse effects, or more rarely, agitation or akathisia or acute suicidality.

Magnesium For Anxiety And Depression

There appears to be a correlation between suboptimal magnesium status among people with depression. A small association was found between low magnesium intake and depression among younger adults and other analyses have found that higher magnesium intakes are associated with lower risk for depression.

Thats telling, but we dont want to hang our hats on survey data from food intake questionnaires. Its not easy to get a solid picture of total body magnesium status, but when measurements are made , low blood magnesium or suboptimal status were found in nearly 40% of subjects in one study.

Special Benzodiazepine Risk Factors

Anyone who takes benzodiazepines can experience unpleasant or dangerous side effects. But certain individuals are at a higher risk:

People over 65. Older adults are more sensitive to the sedating effects of benzodiazepines. Research Instruments And Analyses

Two focus group interview schedules were developed, one for employees and the other for organizational representatives.
