Differential Diagnosis And Comorbidity

Medications for Anxiety Disorders and PTSD: Current Trends and Future Directions

When evaluating a patient for a suspected anxiety disorder, it is important to exclude medical conditions with similar presentations . Other psychiatric disorders use of substances such as caffeine, albuterol, levothyroxine, or decongestants or substance withdrawal may also present with similar symptoms and should be ruled out.5

Complicating the diagnosis of GAD and PD is that many conditions in the differential diagnosis are also common comorbidities. Additionally, many patients with GAD or PD meet criteria for other psychiatric disorders, including major depressive disorder and social phobia. Evidence suggests that GAD and PD usually occur with at least one other psychiatric disorder, such as mood, anxiety, or substance use disorders.10 When anxiety disorders occur with other conditions, historic, physical, and laboratory findings may be helpful in distinguishing each diagnosis and developing appropriate treatment plans.

Who Is At Risk For Post

You can develop PTSD at any age. Many risk factors play a part in whether you will develop PTSD. They include:

  • Your sex women are more likely to develop PTSD
  • Having had trauma in childhood
  • Feeling horror, helplessness, or extreme fear
  • Going through a traumatic event that lasts a long time
  • Having little or no social support after the event
  • Dealing with extra stress after the event, such as loss of a loved one, pain and injury, or loss of a job or home
  • Having a history of mental illness or substance use

Relative Efficacy Of Drugs

In a meta-analysis of all available drug studies in anxiety disorders,77 the pre -post effect sizes of the different drugs were determined. We simply looked at the absolute difference in anxiety scale scores before and after treatment, without regard to the relative efficacy compared with placebo. This approach makes it possible to include hundreds of studies in comparisons of differential efficacy of all available drugs and not only the few direct head-to-head comparisons. From the patients’ point of view, the improvement in anxiety symptoms as measured by the change from baseline to end point is more relevant than the difference from a control group.

The available medications for anxiety disorders showed considerably large differences in pre-post effect sizes. For example, the improvement achieved with the most efficacious drug was almost three times higher than what was accomplished with the drug with the weakest efficacy . Quetiapine, however, is not licensed for the treatment of any anxiety disorder in most countries. Among the drugs showing high effect sizes and that are licensed for anxiety disorders and recommended by guidelines were the SSRIs escitalopram and paroxetine , and the SNRIs venlafaxine and pregabalin . Also, some benzodiazepines, eg, diazepam and lorazepam , showed high effect sizes. However, these drugs are not recommended for routine treatment.

What Are Anxiety Disorders

Anti Anxiety Medication

Anxiety disorders affect 31.9 percent of people between the ages of 13 and 18, and women are twice as likely to develop an anxiety disorder as men. Common examples of anxiety disorders include generalized anxiety disorder, panic disorder, social anxiety disorder, substance-induced anxiety disorder and specific phobias.

An anxiety disorder does more than make you anxious. They can affect your entire life. Plus, untreated anxiety can lead to other mental health disorders and physical conditions, Alonzo said. We have no way of predicting what will cause someone to develop an anxiety disorder. However, you can take steps to reduce the impact of symptoms such as getting help from a health care professional early and avoiding the use of non-prescribed drugs and alcohol.

Acceptance And Commitment Therapy

The second-line treatment for GAD is acceptance and commitment therapy.

In ACT, you learn to accept your thoughts without trying to alter or reduce them.

ACT also helps you focus on the present moment and your surroundings, as well as take action on your values, instead of letting your anxiety dictate your decisions and your days.

  • other medications used off-label, such as beta-blockers

These types of medications are described in more detail below.

Keep in mind that many people with GAD dont respond to the initial medications they try. The next treatment your healthcare provider prescribes will depend on your specific symptoms, treatment history, and preferences.

Do I Have An Anxiety Disorder

Common signs and symptoms of anxiety include nervousness, restlessness or tenseness, a sense of impending danger or doom, increased heart rate, breathing rapidly, sweating, trembling, feeling weak or tired, trouble concentrating or sleeping, stomach aches and headaches and the urge to avoid things that trigger anxiety.

Anxiety disorders often develop from a complex set of risk factors. The risk factors may include exposure to trauma, certain medical problems like thyroid disease or heart disease, stress due to a severe illness and other mental health disorders like depression. The use or misuse of drugs and alcohol can worsen symptoms of anxiety.

Anxiety disorders are best treated with a combination of therapy and medication, Alonzo said. It takes time to find the perfect combination for your symptoms. Your primary care provider is the best place to start that journey.

Selective Serotonin Reuptake Inhibitors

In most cases, the first medication you’ll be offered will be a type of antidepressant called a selective serotonin reuptake inhibitor .

This type of medication works by increasing the level of a chemical called serotonin in your brain.

Examples of SSRIs you may be prescribed include:

  • in men, difficulty obtaining or maintaining an erection

These side effects should improve over time, although some such as sexual problems can persist.

If your medication isn’t helping after about 2 months of treatment or it’s causing unpleasant side effects, your GP may prescribe an alternative SSRI.

When you and your GP decide it’s appropriate for you to stop taking your medication, you’ll normally have your dose slowly reduced over the course of a few weeks to reduce the risk of withdrawal effects.

Never stop taking your medication unless your GP specifically advises you to.

Psychotherapy And Relaxation Therapies

Psychotherapy includes many different approaches, such as cognitive behavior therapy and applied relaxation .33,34 CBT may use applied relaxation, exposure therapy, breathing, cognitive restructuring, or education. Psychotherapy is as effective as medication for GAD and PD.11 Although existing evidence is insufficient to draw conclusions about many psychotherapeutic interventions, structured CBT interventions have consistently proven effective for the treatment of anxiety in the primary care setting.34â36 Psychotherapy may be used alone or combined with medication as first-line treatment for PD37 and GAD,11 based on patient preference. Psychotherapy should be performed weekly for at least eight weeks to assess its effect.

Can Antidepressants Cause Anxiety

Dr. Shelton says that SSRIs typically dont cause anxiety symptoms. However, if a patient is very anxious its a good idea to start on a very low dose to allow the patient to adjust to the medication slowly.

Dr. Plummer says that a patient starting an SSRI may notice an initial increase in anxiety. It takes a minimum of two to four weeks, sometimes longer, for SSRIs to reach the serotonin levels needed to relieve the depression or anxiety, she says.

Mindfulness And Applied Relaxation

Mindfulness and applied relaxation are alternative types of psychological treatment.

Mindfulness focuses your awareness on the present moment. It allows you to acknowledge and accept certain feelings. It can reduce anxiety associated with the fear of actual situations or sensations. It helps to counter the sense of ‘tunnel vision’ that may develop during anxiety.

Applied relaxation focuses on relaxing your muscles in a particular way. A trained therapist teaches the technique which alleviates anxiety and involves:

  • learning how to relax your muscles
  • relaxing your muscles quickly in response to a trigger
  • practising relaxing your muscles in situations that make you anxious

Unresponsiveness To Standard Treatments

TOP MEDICATIONS FOR TREATING ANXIETY

Before considering a patient to be treatment unresponsive, it should be ascertained that the diagnosis was correct, adherence to the treatment plan was sufficient, the dose prescribed had covered the full range, and there had been a trial period of adequate duration. When patients report previous treatment failures, it often turns out that a drug was only prescribed in the lowest dose or was stopped within the first 2 weeks due to side effects that occurred in the initial phase before the patient could experience improvement.

Elderly patients may take longer to show a response. Table III contains options in case of drug inefficacy or intolerance. In patients who are unresponsive to psychotropic drugs, the addition of CBT is generally recommended.85

A combination of antidepressants and benzodiazepines is sometimes used in treatment-refractory cases.

When all standard treatments have failed, the off-label use of drugs may be considered, for example, drugs licensed for another anxiety disorder or that are not licensed but have shown efficacy in clinical studies. Such drugs include quetiapine and agomelatine.

There are several types of medications used to treat anxiety disorders.

Within each of these categories, there are subgroups of drugs that work differently and have their own benefits, risks, and possible side effects.

Theyre supereffective in treating anxiety disorders, says Beth Salcedo, MD, the medical director of the Ross Center for Anxiety & Related Disorders and a past board president of the Anxiety and Depression Association of America. Its not known exactly how SSRIs work on serotonin to alleviate anxiety, says Dr. Salcedo, but what is known is that theyre not addictive like benzodiazepines are .

Its important to follow your doctors instructions when taking any prescription drug. Talk to your doctor if youre concerned about the side effects of a medication, or if the drug youre taking doesn’t seem to be alleviating your symptoms.

The doctor was not able to see her in person at that time. They connected via email for 6 months up until she might go to the medical professional in person once more.

Unanswered Questions And Future Research

A more complex model with incorporation of covariate effects could be implemented in further work to elucidate and therefore adjust for other factors that might have an impact on parameters as these would manage any variation between the direct and indirect evidence. Finally, we identified relatively little published evidence on patient reported outcomes such as quality of life in generalised anxiety disorder. These limitations in the evidence base could form the rationale for future research investigating comparative effectiveness of treatments used in generalised anxiety disorder over the longer term.

What is already known on this topic

  • Generalised anxiety disorder is a common, typically chronic, and impairing medical condition for which a range of drug and psychological treatments are available

  • Recent guidelines tend to recommend a selective serotonin reuptake inhibitor for first line treatment on the basis of benefit in reducing anxiety symptoms with generally mild side effects, but the comparative efficacy and tolerability of treatments is uncertain

What this study adds

  • In this mixed treatment meta-analysis fluoxetine was ranked first for response and remission and sertraline was ranked first for tolerability

  • Among treatments licensed specifically for generalised anxiety disorder in the UK, duloxetine, escitalopram, and pregabalin might offer some advantages over venlafaxine and paroxetine

Selecting A Treatment Approach

When selecting a treatment for patients with GAD, physicians should consider several factors, including patient preference, treatment success history, and other factors that may interfere with successful treatment .66 Psychiatric or psychological referral should be considered in patients who fail to demonstrate improvement or if serious comorbid problems, such as suicidal/homicidal ideation or substance abuse, occur or worsen .

Combining Psychotherapy And Medication

Both psychotherapy and pharmacotherapy have been shown to be more effective than control groups. However, whereas drugs are mostly compared with placebo controls, the evidence for psychotherapy in anxiety disorders is mainly based on comparisons with a wait-list, a control method that was used in 70% to 75% of the studies in adults and children.77,103,104 Because pill placebos have higher effect sizes than waitlist controls, the effect size differences between active and control conditions cannot be compared between psychotherapy and medication studies. Therefore, our research group conducted a large meta-analysis of all available controlled short-term studies for anxiety disorders and compared the pre-post effect size differences between medications and psychotherapies.77 In this meta-analysis, which was based on studies with around 35 000 patients, medications were associated with a significantly higher average pre-post effect size than psychotherapies . It was also found that patients included in psychotherapy studies were less severely ill than those recruited for medication trials.

The meta-analysis also showed that combinations of psychotherapy and pharmacotherapy had a relatively high effect size . However, only a few combination studies were available for this comparison, and some of these have not been conducted with the most powerful drugs.
